A Top-Down Approach to Facial Rejuvenation

Facial ageing does not always begin in the lower face.
As we age, changes can occur across the forehead, brow, eyelids, cheeks, jawline and neck. These areas are connected and can influence the overall balance and appearance of the face.

This is why some facial surgeons take a top-down approach to facial rejuvenation, assessing the upper face before deciding whether the middle and lower face also require treatment.

Brow Lift Techniques

There are several techniques that may be used to perform a brow lift. The most appropriate technique depends on your anatomy, degree of brow descent, hairline, forehead height, previous surgery and desired result.

Endoscopic Brow Lift

An endoscopic brow lift uses several small incisions within the scalp and specialised instruments to lift and reposition the brow tissues.
This technique may be appropriate for some patients and generally involves smaller incisions than a traditional coronal brow lift.

Coronal Brow Lift

A coronal brow lift involves a longer incision within the scalp, generally extending from ear to ear.
This technique can provide significant lifting and repositioning of the forehead and brow but involves a longer incision and is not appropriate for every patient.

Direct Brow Lift

A direct brow lift involves an incision directly above or close to the brow to lift the eyebrow.
This technique may be considered in selected patients where precise brow elevation is required.

Temporal Brow Lift

A temporal brow lift focuses primarily on the outer portion of the brow and temple area.
It may help lift the outer brow and improve the appearance of heaviness or tiredness around the outer eyes.
A temporal lift may be performed on its own or combined with another brow lift technique.
In some patients, combining a brow lift with a temporal lift allows the surgeon to address both the central forehead and outer brow as part of a more comprehensive upper-face rejuvenation plan.

Brow Lift vs Eyelid Surgery

A brow lift and upper eyelid surgery address different areas of the upper face.
A brow lift primarily addresses the position of the eyebrow and forehead.

Upper eyelid surgery, also known as upper blepharoplasty, primarily addresses excess skin and tissue of the upper eyelid.
Sometimes the appearance of heavy or hooded eyes is caused partly by a descended brow rather than simply by excess eyelid skin.

This is why it is important to assess the brow and eyelids together rather than automatically assuming that eyelid surgery is the answer.
Some patients may benefit from a brow lift. Others may benefit from upper eyelid surgery, while some may achieve the best result from combining both procedures.

Your surgeon will assess the cause of the heaviness around your eyes before recommending treatment.

Brow Lift and Temporal Lift

A temporal lift is designed primarily to address the outer brow and temple area. It may be considered when the outer portion of the brow has descended or when there is heaviness around the outer eyes. A temporal lift can sometimes be performed alongside a brow lift to create a more comprehensive improvement across the upper face. The surgeon will assess:

A brow lift primarily addresses the upper face, while a facelift primarily addresses aging and sagging in the middle and lower face, jawline and, depending on the technique, the neck. If aging is present across several areas of the face, your surgeon may recommend combining procedures. This may include:

Brow Lift and Fat Grafting

Facial ageing does not only involve tissue descent. Loss of facial volume can also contribute to an older or tired appearance. Fat grafting may be recommended alongside a brow lift where there is volume loss in selected areas. Fat may be used to restore volume to areas such as:

Fat grafting is not necessary for everyone. Your surgeon will determine whether restoring facial volume would improve your individual result.

Can a Brow Lift Improve Forehead Lines?

A brow lift may improve the appearance of some forehead and frown lines, particularly where these lines are associated with aging and changes in the position of the brow and forehead tissues. However, a brow lift is not primarily a wrinkle treatment. Some forehead lines are caused by repeated muscle movement, while others are related to skin quality, sun damage or volume changes. Can a Brow Lift Make My Eyes Look More Open?

Yes, a brow lift can create a more open appearance around the eyes when brow descent is contributing to upper-face heaviness. The aim is generally to create an appropriate and natural brow position, rather than simply lifting the brows as high as possible. Will a Brow Lift Make Me Look Surprised?

A common concern is that a brow lift may create an unnaturally high or permanently surprised appearance. Modern facial rejuvenation aims to create a natural result rather than simply raising the brows as high as possible. Your surgeon will consider your existing brow position, facial proportions, forehead anatomy and desired outcome when determining the appropriate amount of lift. Brow Lift Procedure

A brow lift may be performed under general anaesthesia or, depending on the surgical technique and individual circumstances, local anaesthesia with sedation. For example, a temporal brow lift may be performed under local anaesthesia with sedation.

The exact procedure will depend on your anatomy, degree of brow descent, surgical technique and whether additional procedures are being performed.

2 Incisions

5 Additional Procedures

If a temporal lift, eyelid surgery, fat grafting, facelift or another procedure is included in your surgical plan, this may be performed at the same time where appropriate.

6 Closure

The incisions are carefully closed and appropriate dressings are applied.

7 Recovery

You are monitored in the recovery area before being transferred to your hospital room. Your Patient Coordinator will be there on the day of surgery to provide support, assist with communication and help ensure everything runs smoothly.

Brow Lift Scarring

A brow lift creates scars at the incision sites. The location and length of the scars depend on the surgical technique used.

With some techniques, incisions are concealed within the scalp or hairline. Other techniques may involve an incision closer to the eyebrow.

Your surgeon will explain the expected incision pattern and scarring during your consultation.

Scars initially appear more noticeable but generally mature and become less visible over time.
