Blepharoplasty, commonly known as eyelid surgery, is a cosmetic procedure designed to address excess skin, tissue and fat around the upper and/or lower eyelids. It can improve heavy or hooded upper eyelids, under-eye bags and other visible signs of ageing around the eyes, creating a more refreshed and natural appearance. However, tired-looking or hooded eyes are not always caused by the eyelids themselves. In some patients, the brow has descended with age and is contributing to heaviness around the eyes. In these cases, a brow lift may be recommended instead of, or in combination with, blepharoplasty. Types of Blepharoplasty
Upper Blepharoplasty
Upper eyelid surgery focuses on excess skin and tissue affecting the upper eyelids. It may be appropriate for hooding, excess skin, heavy-looking eyelids, age-related changes or skin affecting the visual field. The incision is generally placed within the natural crease of the upper eyelid so that the scar is concealed within the natural fold once healed.
Lower Blepharoplasty
Lower eyelid surgery focuses on bags, puffiness and other signs of ageing beneath the eyes. Depending on your anatomy, the surgeon may remove excess skin, remove or reposition fat, reposition existing tissues or address lower-eyelid laxity. The aim is to create a smoother transition between the lower eyelid and cheek rather than simply removing as much fat as possible.
Upper and Lower Blepharoplasty
Some patients have ageing affecting both the upper and lower eyelids. Upper and lower blepharoplasty can be performed together to provide more comprehensive rejuvenation around the eyes.
Your surgeon will determine whether treating both areas is appropriate for you.
Blepharoplasty or Brow Lift?
Blepharoplasty primarily addresses the eyelids, while a brow lift addresses the brow and forehead. A heavy or hooded appearance can be caused by excess eyelid skin, but it can also be caused or worsened by descent of the brow.
When the brow drops with age, it can push tissue down towards the upper eyelid and make the eyes appear tired or hooded. Simply removing eyelid skin may not address the underlying problem.
Your surgeon will assess brow position, forehead, upper eyelids, lower eyelids, facial symmetry and overall facial balance.
You may need blepharoplasty, a brow lift, or a combination of both.
A Top-Down Approach to Facial Rejuvenation
Facial ageing affects the entire face, and the brow, eyes, cheeks, jawline and neck are interconnected. Where appropriate, he takes a top-down approach. Significant ageing or descent in the upper face may be addressed before, or together with, the middle and lower face. This may involve a brow lift, temporal lift, blepharoplasty, fat grafting, facelift or neck lift.
This approach considers how different areas of the face have changed and the sequence in which they may need to be treated.
The goal is not simply to remove skin or pull the face tighter. It is to create a balanced and natural result.
Blepharoplasty and Fat Grafting
Ageing around the eyes is not always caused by excess skin. Loss of facial volume can also contribute to a hollow or tired appearance.
Where appropriate, fat grafting may be recommended alongside blepharoplasty to restore volume and improve overall facial balance. Fat may be transferred to the temples, upper cheeks, areas around the eyes or other areas where volume has been lost.
Fat grafting is recommended only where it is appropriate for your anatomy and goals.

During your consultation in Thailand, your surgeon will:

Blepharoplasty Procedure
Our standard upper and lower blepharoplasty package is performed under local anaesthesia as an outpatient procedure. You will attend the hospital for your surgery but do not require an overnight hospital stay. The exact procedure depends on whether you are having upper, lower or combined eyelid surgery.
1. Incisions
For upper blepharoplasty, the incision is generally placed within the natural eyelid crease. For lower blepharoplasty, the incision may be placed beneath the lower eyelashes or, where appropriate, inside the lower eyelid.
2. Tissue Assessment
The surgeon assesses the skin, fat and underlying tissues.
3. Skin and Fat Treatment
Excess skin may be removed and fat may be removed or repositioned depending on your individual anatomy.
4. Tissue Repositioning
Where appropriate, tissues may be repositioned rather than simply removed to help create a natural contour.
5. Closure
The incisions are carefully closed with fine sutures.
6. Recovery
You will be monitored following your procedure before being discharged from the hospital. Your Patient Coordinator will assist with your arrangements and transfers. If additional procedures such as a brow lift, facelift or other surgery are included, the anaesthetic and hospital arrangements may be different.
Recovery After Blepharoplasty
Because standard blepharoplasty is performed as an outpatient procedure, there is no overnight hospital stay.
You may experience swelling, bruising, tightness, mild discomfort, temporary numbness, temporary sensitivity around the eyes and temporary changes in sensation.
Swelling and bruising are generally most noticeable during the early stages of recovery and gradually improve. Cold compresses may be recommended. Light walking is encouraged, while strenuous exercise, heavy lifting and activities that increase swelling should be avoided until your surgeon confirms it is safe.
Blepharoplasty Scarring
Blepharoplasty creates scars at the incision sites. For upper blepharoplasty, the incision is generally positioned within the natural crease of the eyelid.
For lower blepharoplasty, the incision may be placed just below the eyelashes or, with some techniques, inside the eyelid.
Scars initially appear more noticeable but gradually mature and generally become less visible over time.
Your surgeon will explain the expected incision pattern and scarring during your consultation.
Laser Treatment After Blepharoplasty
Our surgeons generally recommend allowing the tissues to fully heal before undergoing laser treatment around the eyes or face.
Laser treatment is generally recommended around 6 months after surgery, depending on the type of treatment and your surgeon's advice.
Your surgeon will advise you when it is appropriate to resume or commence laser treatments.
